Dr. Hasen performs breast augmentation surgery under general anesthesia in our fully-accredited, private surgery suite in Naples, Florida. Since there are a number of incision choices, Dr. Hasen gives you the choice of placement. Most patients choose either an incision in fold at the bottom of the breast or around the lower portion of the areola. In late 2006, the FDA approved the use of silicone gel filled breast implants for cosmetic enhancement in women 22 years of age and older.

Saline or Silicone?

Implants can be filled with either saline solution or silicone gel material. The FDA approved the use of moderately cohesive silicone gel filled breast implants in November, 2006. The fully or highly cohesive gel filled breast implants (sometimes referred to as "gummi bear" implants) are not yet FDA approved. There are benefits and drawbacks for each implant fill type. The main advantage of silicone breast implants over saline is that there is less palpability, wrinkling and ridging of the implant. For this reason, silicone implants feel and look more natural. Additionally, they can be placed in a more superficial plane (i.e. under the breast, but above the muscle) without the same issues of visibility as saline breast implants. The main drawbacks of silicone implants are that they are more expensive, harder to detect shell ruptures and require larger incisions since they are pre-filled with the moderately cohesive gel material. The main advantages of saline breast implants are that they can be placed through a smaller incision, the size can be adjusted intra-operatively, they are less expensive and ruptures are very obvious.

The goal of the breast augmentation surgery is an end result that improves the appearance of your breasts with better shape and proportions that helps improve your self image.

Above or Below the Muscle?

Implants can be placed either above or below the chest muscle (the pectoralis major). When an implant is placed under the pectoralis muscle, it mainly helps hide the implant edges in the top and medial aspect of the breast. Because the muscle is fan shaped, it does not cover the implants outside edge. Furthermore, the bottom of the implant is not covered by the muscle either because the lower edge of the muscle has to be lifted off the underlying rib during the surgery to allow the implant to sit low enough. Thus, the problem areas of palpability and skin wrinkling or rippling in cases of implant placement under the muscle usually occur on the side and/or the bottom of the breast. The main drawbacks of placing the implant under the muscle are the pain at the initial surgery and variable degree of motion of the implant and breast that occurs with contraction of the chest muscle that can be quite bothersome to patients. Above the muscle allows for less pain at the time of the surgery and no motion with contraction of the chest muscle.

Again, usually only silicone implants are suitable for placement above the muscle. The FDA warns that placement of the implant above the muscle may impair mammography more than below the muscle. Many mammographers and plastic surgeons contend this point. Many radiologists feel that the biggest limiting factor to getting reliable mammography with implants is the presence of a capsular contracture regardless of the breast implant being above or below the chest muscle. Mammography in patients with significant capsular contracture of the implants is difficult because the firm implant and capsule are adherent to the breast tissue and do not allow for displacement of breast away from the implant for the mammogram.

Who Is a Good Candidate?

  • Patients who have small, uneven or asymmetric breasts.
  • Patients who have slightly sagging or deflated breasts.
  • Patients whose breasts are out of proportion to the rest of the body.
  • Patients who desire a better fit in clothing and a more sensual body image.

The Recovery

  • You can go home from the surgery suite recovery room after an hour or two in the company of an adult who will look after you for the first few days after your cosmetic surgery.
  • You will be in a post-operative surgical bra and/or breast stabilizing band with gauze pads on the incisions immediately after the breast augmentation surgery.
  • You can shower on the first day after surgery.
  • Most patients experience moderate pain for the first two days that is mostly gone by the third day and easily controlled by the medications prescribed by Dr. Hasen.
  • You will experience swelling that is mild to moderate and mild bruising for the first week. This rapidly disappears in the weeks following the breast augmentation surgery.
  • Typically, the breast implants appear high on the chest for the first three weeks after the breast enhancement surgery before they settle into a normal position and contour.
  • You can resume many activities within the first week, and most by the third week.
  • Your sutures may need to be removed at seven to ten days.
  • You may need to be off of work for five to seven days, but often patients take off less time.

The Result

  • Enlarged breasts that appear full and balanced.
  • Very natural result that just improves with time.
  • Softness and natural bounce that persists.
